Job summary

Pathways Hospice is seeking a Registered Nurse Case Manager for a full-time role covering the Northwest, Southwest & Southeast Houston areas. You will lead the nursing care team, collaborate with physicians and therapists, plan home health services, and educate patients and their families to improve outcomes.

The position requires an TX RN license and hospice experience; HomeCare HomeBase EMR experience is a plus. Competitive benefits and on-call rotation are included.

Qualifications

• RN license in Texas is required; hospice experience preferred.
• Knowledge of home health regulations and compliance is important.
• Experience with HomeCare HomeBase (HCHB) EMR is preferred.

Responsibilities

• Oversee nursing care team and coordinate patient care.
• Communicate with physicians, therapists, and healthcare professionals to ensure cohesive care.
• Plan, organize, and direct home health services.
• Conduct patient assessments and develop care plans.
• Educate patients and families on health management and medications.
• Prepare timely clinical notes and maintain documentation.
• Night and weekend coverage required; on-call rotation weekly.

Hospice - RN Case Manager

Shift Available: Full Time

Responsibilities:

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients
• If working the weekend-will receive the following Monday off (Admissions Included)
• Must be willing to participate in an On Call Rotation (Weekly)

Qualifications:

• Valid RN license in the State of Texas.
• Hospice Experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HomeCare HomeBase (HCHB) EMR (preferred).
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.
