Job summary

Candidates must hold a VT/compact RN license, BLS, and have at least 1 year of home health hospice case management experience, including Oasis system proficiency.

POSITION DUTIES

Assesses patients, develops and coordinates individualized hospice care plans, manages symptoms and pain, provides nursing care and education, collaborates with the interdisciplinary team, and supports patients and families to ensure comfort and quality of life during end-of-life care in the home setting.

MINIMUM REQUIRED QUALIFICATIONS –

• VT/compact RN license
• BLS
• 1+ year home health hospice case management experience
• Oasis experience

LENGTH OF ASSIGNMENT

16 weeks

SHIFT / HOURS PER WEEK

Days, M-F with some weekend coverage
