Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of home care and perform patient admissions at the home. You will work with the medical team to determine eligibility for home care services and develop the plan of care to be followed.

Qualified graduates with RN licensure and CPR/Hepatitis clearance are needed, along with a valid driver's license and reliable transportation.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care (home health RN case manager).
• Perform admission visit to the patient in the home and determine eligibility for home care services.
• Develop and follow the plan of care with input from patient and medical team.
