Job summary

Amedisys is seeking an RN Case Manager for Home Health in Missouri. This full-time daytime role coordinates care for patients at home, conducts assessments, and collaborates with the care team to develop effective plans of care.

You will supervise LPNs and HHAs, ensure timely documentation, and communicate progress to the clinical manager. The organization offers a robust benefits package and a generous sign-on bonus.

Qualifications

• One year of clinical RN experience; if less, VP approval may be required.
• Current RN license for the states you will serve.
• Current CPR certification is required.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and appropriate rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
