Job summary

UnitedHealth Group is seeking a Nurse Case Manager in the United States. You will guide patients and families through the care continuum, assess needs, coordinate care, support discharge planning, and manage outpatient care under supervision of an RN or MD.

You will educate, coach, and provide treatment-decision support while collaborating with Care Advocates and Social Workers. This role requires active RN license, 1+ year of RN experience, CPR certification, and a valid driver's license.

Qualifications

• Current RN license for the state(s) assigned.
• Minimum 1 year of clinical RN experience.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes timely and accurate documentation.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and participates in continuing education.
