Job summary
Ironside Human Resources in Salem, Oregon is seeking an RN Case Manager to join its clinical team. You will complete patient assessments and develop individualized care plans while coordinating with physicians and interdisciplinary teams.
The ideal candidate holds an Oregon RN license, has at least one year of acute care experience, and current BLS/CPR certification. A valid driver's license and auto insurance are required, with home health experience preferred.

Responsibilities
- Complete patient assessments and develop individualized plans of care.
- Monitor patient progress and revise care plans as needed.
- Administer medications and treatments according to physician orders.
- Educate patients and families regarding health conditions and care plans.
- Coordinate care with physicians and interdisciplinary healthcare teams.
