Alternate Solutions Health Network in Troy, MI is seeking a Registered Nurse Case Manager to lead home-based care coordination under physician direction, delivering one-on-one patient care and guiding families through recovery.

You will collaborate with an interdisciplinary team, perform Oasis assessments, educate families, and ensure safety and adherence to Medicare/Federal guidelines. A supportive team and flexible scheduling accompany a competitive benefits package.

Qualifications

• Must have Registered Nurse license in state of employment.
• Minimum 1 year RN experience in acute care setting.
• Home care experience preferred.
• Valid driver's license and auto insurance in your name.

Responsibilities

• Lead assessment of patient condition and plan of care.
• Case manage and provide clinical care under physician direction.
• Educate patients and families and ensure patient safety.
• Work autonomously and manage time effectively.
• Follow state scope of practice and agency policies.
• Coordinate with interdisciplinary team for optimal care.
