Job summary

Redcrest Home Health in Indiana is seeking a dedicated Registered Nurse Case Manager to lead patient assessments and care plans across the home health continuum. You will coordinate multi-disciplinary teams, monitor patient progress, complete OASIS assessments, and ensure documentation compliance under physician direction.

At least one year of RN experience, with a preference for home care, is required. Schedule: Monday-Friday 8a-5p with on-call requirements of 5 on-call days per month; travel

Qualifications

• Minimum of one year of RN experience in an acute care setting; home care experience is desired.
• Ability to communicate effectively and project a caring, professional image.
• Compassionate, patient-focused and capable of working with families.
• Knowledgeable about nursing best practices and clinical decision-making.

Responsibilities

• Oversees the overall care of the patient from beginning to end of episode.
• Updates doctor orders in patient chart and notifies physicians of changes.
• Performs OASIS assessments to develop and adjust the plan of care.
• Provides skilled nursing care as ordered by the attending physician.
• Ensures accurate documentation and compliance with Medicare/Federal guidelines.
