Beacon Home Health is seeking an RN Case Manager to join the Home Health team in Cordova, TN. The role covers full delivery of patient care plans and requires coordination with physicians, families, and the IDG to improve care quality.

Responsibilities include visiting patients, documenting visits, and maintaining compliance with agency policies. The position offers full-time benefits and an opportunity for internal advancement.

Qualificações

• One year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification, driver’s license, vehicle insurance, and access to dependable vehicle or public transport.

Responsabilidades

• Provide full delivery of the plan of care for each home health patient.
• Complete documentation accurately and timely (visit reports, OASIS, plan of care, progress notes, etc.).
• Attend meetings for all patients and provide input on care and progress.
• Adhere to agency policies, procedures and processes; seek guidance when needed.
• Establish and maintain an efficient visit route and coordinate with the IDG.
