Job summary

OneWell Health Care is seeking a dedicated RN Case Manager in North Charleston, SC to coordinate and oversee nursing services for individuals with IDD receiving home and community-based care. The role requires regulatory compliance, care coordination, and clinical oversight within SC nurse practice standards.

This full-time position is based in an office setting with frequent travel throughout designated SC service areas.

Qualifications

• Minimum two years of RN experience.
• Current SC RN license in good standing.
• CPR/BLS certification.

Responsibilities

• Coordinate nursing services for IDD recipients in home/community settings.
• Develop and update individualized care plans.
• Ensure regulatory compliance and accurate documentation.
