Job summary

CenterWell Home Health in Columbia, SC is seeking a Home Health Registered Nurse to provide one-on-one, in-home care under the Clinical Manager. You will work with physicians and clinicians to develop and manage care plans that support recovery and help patients regain independence.

You will monitor patients, educate families on disease management and medications, and ensure accurate documentation. A valid RN license and CPR are required, along with a valid driver's license for home visits.

Qualifications

• Med surg, ICU, ER, or acute experience.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients in their homes, including medications, wound care, and assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to patient's physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor their documentation.
