Job summary

Optum, in partnership with Amedisys, is hiring a Nurse Case Manager in Charleston, SC. You will guide patients and families through care coordination, assess needs, support discharge planning, and manage outpatient care under supervision of an RN or MD.

Required qualifications include an active RN license in the state, 1+ year of clinical RN experience, CPR certification, and valid driver's license with reliable transportation.

Qualifications

• Active RN license in the state(s) you will work.
• 1+ year of clinical RN experience.
• CPR certification and valid driver's license with reliable transportation.

Responsibilities

• Assess patient needs and develop care plans with the care team.
• Coordinate referrals and document rationale for actions.
• Educate patients and promote independence through self-care.
• Supervise LPNs and HHAs.
• Complete timely and accurate documentation.
• Communicate patient progress to the clinical manager and care team.
• Plan staff education.
• Participate in on-call responsibilities and continue education programs.
• Pursue continuing education and professional development.
• Other duties as assigned.
