Job summary

Altra Home Health is seeking an RN Case Manager to provide direct care and case management for a client team in accordance with state Nurse Practice Act and agency policy. You will lead RNs, LPNs, Home Health Aides, and Personal Care Assistants to deliver high-quality home care.

Requires a nursing degree, active RN license, and at least 2 years of nursing (home care preferred). Hybrid work location in Indianapolis; salary range $75,000–$85,000 per year with full benefits.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license in state of practice; Baccalaureate degree preferred.

Responsibilities

• Review client referrals and respond to inquiries as appropriate.
• Perform initial and ongoing client assessments per policy.
• Direct and coordinate actions in home care settings.
• Supervise team of RNs, LPNs, and aides to ensure quality care.
• Participate in human resource management to ensure service quality.
• Promote safety and adherence to agency policies.
