Job summary
Mission Healthcare in Salem, OR is seeking an experienced registered nurse to provide in-home care and hospice support within our Salem area territory. You will perform initial assessments, develop and revise care plans, and coordinate with families to meet patient goals.
The ideal candidate holds a current RN license, at least 1 year of acute care or home health experience, and BLS/CPR certification. A valid driver's license and insurance are required; relocation assistance is available for
Qualifications
- Current and valid state RN license.
- Minimum of 1 year of recent acute care experience in an institutional setting.
- BLS with CPR certification.
- Valid driver's license and auto insurance.
- Associates or Bachelor's degree in Nursing; 1 year home health experience preferred.
Responsibilities
- Complete initial patient and family assessment to determine home care needs.
- Initiate and revise the plan of care as patient status changes.
- Regularly re-evaluate patients’ needs and document nursing diagnoses.
- Develop care plans with goals based on nursing diagnosis and involve family.
- Administer medications and treatments as prescribed.
- Counsel patient and family on plan of care and discharge planning.
