Job summary

Visiting Nurse & Hospice of Litchfield County is seeking compassionate Registered Nurses for our Home Health and Hospice teams in Connecticut. You will perform in-home assessments, develop care plans, and coordinate with physicians to deliver patient-centered care.

Qualifications

Active Connecticut RN license is required.
Current CPR certification is required.
Valid driver's license and reliable transportation required.
Strong communication and assessment skills required.
Minimum of one year of home care and/or hospice experience preferred.

Responsibilities

Perform comprehensive nursing assessments.
Develop and manage individualized care plans.
Provide skilled nursing care in patients’ homes.
Educate patients and caregivers.
Collaborate with physicians and interdisciplinary team members.
Document patient care accurately and efficiently.
Respond to changing patient needs with clinical judgment and compassion.
