Job summary

RVNAhealth is seeking an RN Case Manager to lead clinical care for home health patients across Fairfield and Litchfield Counties. You will coordinate care from admission through discharge, partnering with physicians, therapists, social workers, and caregivers in patients' homes.

The role requires an accredited nursing degree, CT RN license (or compact), and at least 1 year of nursing experience, with home health or OASIS experience preferred. Travel across assigned territories is expected.

Qualifications

• Must be a graduate of an accredited School of Nursing.
• Current Connecticut RN license or Compact RN License.
• Minimum of one year of nursing experience.

Responsibilities

• Perform Start of Care visits, assessments, and ongoing evaluations.
• Develop and manage individualized plans of care based on physician orders and patient goals.
• Provide skilled nursing care and case management for adults in their homes.
