Job summary

Enhabit Home Health & Hospice is seeking dedicated clinicians to provide coordinated home-based care, assessing patient and family needs and implementing the interdisciplinary plan of care. You will perform services per physician orders and ensure compliance with federal and state regulations.

Candidates should be a graduate of an approved school of clinical education, licensed in the practicing state, with at least one year of licensure experience and basic tech skills, including mobile device

Qualifications

• Must graduate from an approved school of clinical education.
• Licensed in the state where you currently practice.
• At least one year of experience as a licensed professional.
• Basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient’s and family’s needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.
