Job summary

Enhabit Home Health & Hospice is seeking compassionate clinicians to coordinate care for patients in the home setting. You will assess needs, implement the care plan with the interdisciplinary team, and ensure services comply with physician orders and regulatory requirements.

Candidates must be a graduate of an approved clinical education program, be licensed in Texas, have at least one year of licensed professional experience, and demonstrate basic technology skills including mobile devices.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Provide coordination of care and continuous assessment of patient and family needs.
• Implement the interdisciplinary care plan and follow physician orders.
• Ensure delivery of care complies with federal and state regulations.
