Job summary

Dartmouth Health Home Care is seeking an RN Case Manager to join our patient-centered home health team serving Vermont and New Hampshire. The role blends independent patient visits with collaboration within an interdisciplinary team to deliver high-quality care.

You will develop and adjust care plans, coordinate with physicians and caregivers, document progress, and mentor LPNs/LNAs. A VT or NH RN license (compact acceptable) and a valid driver’s license to travel to patient homes are required.

Qualifications

• Must be a licensed RN in Vermont and New Hampshire or hold a compact license.
• Graduate of an accredited school of nursing.
• One year of experience as an RN in a home health care setting is preferred.

Responsibilities

• Develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission.
• Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.
• Coordinate the patient's care plan through interdisciplinary communication and case conferences, as appropriate.
• Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals.
• Document evaluations, treatment goals, and objectives.
• Teach the patient and caregiver self-care techniques as appropriate; provide instructions as ordered by the provider.
• Provides clinical oversight of LPNs and LNAs in the home.

Responsibilities

The RN Case Manager at the direction of the Clinical Manager, screens, evaluates, re-evaluates and treats patients referred by a licensed provider. Coordinates and communicates treatment activities with health care team members and the patient. Works closely with the interdisciplinary team to ensure high-quality patient care.

• Develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission.
• Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.
• Coordinate the patient's care plan through interdisciplinary communication and case conferences, as appropriate. Advise other disciplines of discharges in a timely manner.
• Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals. Communicate significant changes to the patient's physician and DHHC colleagues.
• Document evaluations, treatment goals, and objectives.
• Teach the patient and caregiver self-care techniques as appropriate. Provide medication, diet, and other instructions as ordered by the provider and utilizes opportunities for health counseling with patients and caregivers.
• Provides clinical oversight of LPNs and LNAs in the home.

• Graduate of an accredited school of nursing

Required Licensure/Certifications

• Must hold at least one active unencumbered RN license in either unencumbered VT or NH at the time of hire. Employees must have active unencumbered RN licenses in both VT and NH within 30 days of their hire date.
• Valid driver’s license with a clean driving record
• Current car insurance, which meets minimum standards
• BLS within 90 days of hire
