Job summary

HOMECARE WITH HEART, LLC is seeking a Registered Nurse Case Manager to provide in-home nursing care for patients in the Youngstown area. The role requires coordinating care with physicians, therapists, and social workers to develop and implement patient care plans.

This position serves Trumbull County, with opportunities for full-time or part-time work. The ideal candidate will have an active RN license, CPR certification, and prior home health experience.

Qualifications

• Nursing supervisory/leadership experience preferred.
• Six months' experience in home care preferred.
• Active RN license and CPR certification.

Responsibilities

• Assess, plan, direct and evaluate each patient’s care plan to ensure it meets the patient’s needs.
• Manage emergency situations promptly and professionally.
• Provide nursing care in home settings and maintain progress notes.
• Participate in branch on-call rotation and adapt to changing patient needs.
• Utilize the Electronic Visit Verification system and coordinate with the care team.
