Qualifications

• Active Wisconsin Registered Nurse License.
• Two years nursing experience; 1 year in Home Health preferred.
• CPR Certification required.
• Valid Wisconsin driver's license and car insurance.

Responsibilities

• Coordinate and oversee patient care in the home setting.
• Provide skilled nursing care including assessments, medications, and treatments.
• Counsel patient and family in the home per physician orders and plan of care.
