Job summary

Team Select Home Care is seeking a qualified Registered Nurse Case Manager to oversee and coordinate patient care per physician orders and the plan of care. You will report to the Nursing leadership and supervise staff while ensuring compliance with state and payer requirements.

You will develop individualized care plans, manage data collection, and advocate for patient welfare across multiple care settings.

Qualifications

• Graduate of an approved nursing program and licensed in the state(s) practiced.
• Minimum 1 year nursing experience; home health experience preferred.
• Ability to perform required data collection and documentation per payer/state.
• Excellent communication and critical thinking for patient care.

Responsibilities

• Develops, delivers, implements, revises, and evaluates individualized patient-centered care plans under physician supervision.
• Provides case management to assigned patients and staff.
• Documents data collection sets (OASIS, PAR) within required timeframes.
• Initiates and reevaluates nursing needs per state and payor requirements.
• Coordinates care among patients, families, agencies, and providers.
• Administers medications per policy and orders and ensures patient safety.
