Job summary

Western CT Home Care Inc. seeks a Registered Nurse to support home health clients, assess needs, and implement intervention plans within agency policy. You will collaborate with physicians, patients, and families to coordinate care and monitor outcomes.

Qualifications

• Strong communication and research skills.
• Knowledge of home health policies and compliance.

Responsibilities

• Assess physical, emotional, socio-economic and environmental needs of patients and families.
• Provide physician-ordered services per the plan of care and coordinate with other practitioners.
• Develop, implement, and evaluate the care plan with patient, caregiver, and physician.
• Coordinate services with patient and family; provide education and counseling.
• Regularly evaluate progress and escalate changes per policy.
• Participate in quality assurance and professional training programs.
• Identify needs for other home health services and referrals.
• Supervise homemaker/home health aide and report evaluations to supervisor.
• Fulfill compliance duties, prepare clinical notes, attend training.
• Perform other duties as assigned.
