Job summary

Amedisys, part of the Optum family, is hiring a Nurse Case Manager in Warwick, RI. You will guide patients and families through the care continuum, assess needs, coordinate care, support discharge and transition planning, and manage ongoing outpatient care under supervision.

You will educate, coach, and support treatment decisions while collaborating with a Care Advocate and Social Worker. The role includes on-call duties and opportunities for professional development within a supportive team.

Qualifications

• Current RN license for the state(s) assigned.
• Current CPR certification required.
• 1+ year of clinical experience as an RN.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as needed and documents rationale if not.
• Promotes patient health and independence through teaching and rehabilitation.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services.
