Qualifications

• 1+ year of clinical experience as an RN. If less than 1 year, VP approval required.
• Current RN license for the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Conduct patient assessments and collaborate with care team to implement a plan of care.
• Make referrals as needed based on patient needs.
• Promote patient health and independence through teaching and rehabilitation.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Participate in on-call duties for patients and families.
• Engage in ongoing clinical development and education Programs.
• Perform other duties as assigned.
