Job summary

Interim HealthCare Inc. is seeking a Registered Nurse Case Manager (RNCM) to conduct in-person patient interviews and comprehensive assessments, and to oversee the care plan through a team of caregivers.

The role emphasizes patient-centered care and collaboration with providers to adapt to evolving needs. Qualified candidates hold an Indiana RN license with CPR/BLS/First Aid, have 1 year of home health or similar experience, and preferably OASIS experience.

Qualifications

• RN license in Indiana.
• CPR/BLS/First Aid certification required.
• 1 year RN experience in home health or similar setting; case management experience preferred.
• OASIS experience preferred.

Responsibilities

• Conduct in-person patient interviews and comprehensive physical assessments.
• Oversee the plan of care with home health aides and other caregivers.
• Communicate patient conditions and collaborate with providers.
