Qualifications

Responsibilities

• Conduct comprehensive in-home patient assessments to determine care needs.
• Develop and document individualized care plans based on assessment findings.
• Evaluate health status, functional abilities and home safety.
• Collaborate with physicians, care teams and families to coordinate services.
• Ensure accurate clinical documentation in compliance with regulatory standards.
• Monitor and reassess patient conditions and adjust care plans as needed.
• Advocate for patients and connect them with community resources.

• Ensure accurate and timely clinical documentation in compliance with regulatory standards
• Monitor and reassess patient conditions, adjusting care plans as needed
• Serve as a patient advocate, connecting clients with community resources and support services

Requirements:

• Active and unrestricted Registered Nurse (RN) license in the state of Florida
• Prior experience in home health, case management, or community health nursing preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills with patients and families
• Ability to work independently and manage a flexible schedule in the field
• Reliable transportation and valid driver's license required
• Proficiency with electronic health records (EHR) and clinical documentation
