Qualifications

Responsibilities

• Conduct comprehensive in-home patient assessments to determine care needs and eligibility.
• Develop and document individualized care plans based on assessment findings.
• Evaluate patient health status, functional abilities, and home environment safety.
• Collaborate with physicians, care teams, and family members to coordinate services.
• Ensure accurate and timely clinical documentation in compliance with regulatory standards.
• Monitor and reassess patient conditions, adjusting care plans as needed.
• Serve as a patient advocate, connecting clients with community resources and support services.
