Job summary

Angels Care Home Health in Liberty, TX is seeking a Registered Nurse Case Manager to coordinate patient home care and perform admission visits in the patient’s home.

You will determine eligibility for home care services, develop the plan of care, and collaborate with other medical disciplines to ensure quality patient outcomes. A valid RN license, driver’s license, and reliable transportation are required.

Qualifications

• Graduate of an accredited diploma, associate or baccalaureate nursing program.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year RN experience; two years preferred.
• CPR and Hepatitis consent/declination.

Responsibilities

• Coordinates all aspects of the patient’s home care.
• Interacts with other medical disciplines as needed and provides professional care.
• Performs admission visit to the patient in the home.
• Determines patient eligibility for home care services and develops the plan of care.
