Trinity Health PACE seeks an experienced nurse leader to coordinate nursing services for participants in the PACE program. The role involves supervising center and in-home staff, partnering with the interdisciplinary team, and ensuring regulatory compliance.
The position requires a flexible schedule, including every other weekend rotations and on-call duties. You will engage in care planning, QAPI initiatives, budget planning, and staff development to maintain high-quality, patient-centered care
The primary purpose of your job position is to coordinate and supervise nursing services provided to the participants in the PACE Organization (PO). Services may be delivered in the day center or in participant's home.
As the programs grows, rotating schedule of every other weekend may be required. Administrative on call will be required about every 6 weeks (no on-site reporting). Position will require work assignments in the facility as well as home care assignments.

Pay Range: $39.81-$49.76/hr
