Job summary

American Health Partners seeks an RN Case Manager to coordinate care for Medicare Advantage members. You will visit patients in person or by telehealth, assess needs, initiate and revise care plans, and collaborate with the health care team to ensure optimal patient outcomes.

Ideal candidates have 2+ years in clinical nursing or geriatric rehab, 2 years in managed care, and 3–5 years in case management within a managed care setting. Strong communication and organizational skills are essential.

Qualifications

• Minimum of 2 years of experience in clinical nursing or rehabilitation for the geriatric population.
• 2-years managed care experience required.
• Minimum of 3-5 years experience doing case management in a managed care environment, preferred.

Responsibilities

• Complete Health Risk Assessments for members as assigned.
• Initiate, update and revise care plans as needed.
• Maintain a case load of patients as assigned.
• Coordinate outpatient discharge planning based on patient needs, clinical circumstances and benefit coverage.
• Participates in Managed Care audits and maintains reports via MS Office.
