American Health Partners is seeking an RN Case Manager to oversee care coordination for Medicare Advantage members. You will visit patients in person or by telehealth, assess nursing needs, and collaborate with physicians to ensure accurate care plans and proper documentation.

The role requires an independent, team-oriented self-starter who can manage multiple priorities, communicate effectively, and work with and across care teams.

Qualifikationen

• Graduate of an accredited RN program.
• Minimum of 2 years clinical nursing or geriatric rehab experience.
• 2+ years managed care experience preferred.
• Current RN license and CPR certification.

Aufgaben

• Complete Health Risk Assessments for members as assigned.
• Initiate and revise care plans; maintain caseload.
• Coordinate with interdisciplinary team to plan and evaluate care.
• Educate patients and families using available resources.
• Coordinate discharge planning and support audits and reporting.
