UniversaCare Inc. is seeking a full-time RN Case Manager in Waterbury, CT and surrounding areas. You will assess patients, develop care plans, perform OASIS assessments, and coordinate care and discharge planning to support a patient-centered home care team.

The role requires an active CT RN license, CPR certification, and a minimum of 1 year RN experience (home care a plus). A valid driver’s license and reliable vehicle are needed.
