Responsibilities

• Coordinate and manage care for patients across the care continuum.
• Conduct comprehensive patient assessments to identify needs.
• Develop and evaluate individualized care plans with teams.
• Monitor progress and adjust care plans as needed.
• Advocate for residents and ensure communication among care providers.
• Facilitate referrals, appointments, and access to resources.
