Qualifications

• RN license in good standing with state board.
• ADN required; BSN preferred.
• Experience in case management or utilization review preferred.
• Direct patient care experience strongly preferred.
• Excellent communication and problem-solving skills.

Responsibilities

• Coordinate and manage care for patients across the care continuum.
• Conduct comprehensive patient assessments.
• Develop and evaluate individualized care plans.
• Monitor progress and adjust plans as needed.
• Advocate for residents and ensure follow-up.
• Facilitate communication among patients, families, and providers.
• Coordinate appointments, referrals, and transportation.
• Ensure appropriate utilization of resources while maintaining high standards.
• Educate patients and families about diagnoses and options.
