Job summary

This is an on-site role with collaboration across physicians and care teams.

Qualifications

• Discharge planning and case management experience in a medical setting.
• Nursing background with Florida licensure or eligibility.
• 1–2 years of discharge planning, case management or managed care experience.

Responsibilities

• Participate in development of patient's plan of care and throughput.
• Conduct utilization review and promote appropriate utilization of resources.
• Coordinate discharge planning and collaborate with care team and families.
