JRA Health in Jacksonville, FL seeks an experienced RN Case Manager to coordinate discharge planning, utilization management, and personalized care plans that improve patient outcomes through collaboration with interdisciplinary teams.

This on-site role requires strong clinical assessment, patient education, and seamless handoffs with intake, operations, and home health partners; prior authorization support and IV setup may be involved.

Qualificações

• Minimum 3 years of inpatient clinical case management experience.
• Registered Nurse Licensure in Florida.
• Proficiency with Microsoft 365 environment (Excel, Word, Teams, Outlook, PowerPoint).

Responsabilidades

• Onboard and transition patient infusion care safely with intake & operations.
• Collaborate on prior authorization clinical reviews and submissions.
• Assist billing with troubleshooting ICD-10 claim denials.
• Participate in quality assurance post-admission assessments and care planning.
• Provide bedside education and IV hookup support as needed.
