Job summary

The University of Miami UHealth Department of UMHC-SCCC is seeking a full-time Case Manager RN to coordinate the interdisciplinary plan of care from admission to discharge, monitor care, and liaise with patients, families, clinicians, and insurers to ensure timely, appropriate, and cost-effective services.

The role focuses on discharge planning, identifying barriers to care, evaluating plans through chart reviews and interviews, educating patients, and ensuring compliance with medical advice

Qualifications

• Bachelor’s degree in nursing.
• Active RN license in Florida.
• Minimum 2 years of relevant experience.
• Strong communication and analytical skills.

Responsibilities

• Identifies risk factors and obstacles to care and discharge.
• Evaluates plan of care regularly via chart reviews and patient interviews.
• Educates patients and families on progression of care.
• Acts as liaison between patients, families, and healthcare personnel.
• Promotes quality care and ensures medically appropriate services.
• Facilitates insurance authorization and discharge planning.
