As a member of a multidisciplinary care management team, the nursing case manager is responsible for coordinating care and services for selected patient populations.

Responsibilities

• Assess patient goals, including biophysical, psychosocial, environmental, discharge planning needs, and financial status.
• Monitor patient progress for changes affecting treatment.
• Procure services and advocate for patients and families.
• Collaborate with third-party payers to ensure continuity of care.
• Coordinate discharge planning with families, healthcare teams, and community resources.
• Review admission data to determine appropriate level of care.

Qualifications

• Graduate from an accredited nursing program; BSN or higher preferred.
