Job summary

• RN license in Ohio; BSN preferred or must be obtained within 3 years.
• Minimum 3 years of clinical experience; Case Management certification preferred.
• Case Management certification required within 3 years of hire date.
• Knowledge of DRG assignment and post-acute transitions.

Responsibilities

• Manage patient transitions of care through the hospital to the appropriate post-acute setting.
• Secure third-party payer approvals for services provided.
• Ensure documentation supports patient status, level of care and DRG.
