Job summary

Mercy Health in Cincinnati seeks an RN Care Manager to coordinate patient care across the continuum, ensuring safe and timely transitions. The role requires assessment, planning, implementation and evaluation of services to meet health and social needs.

Ideal candidates hold a BSN, an active RN license, and relevant certifications. Experience in discharge planning and post-acute care coordination is preferred; collaboration with a multidisciplinary team is essential.

Qualifications

• Bachelor of Science in Nursing (BSN) preferred for BSMH, required for RSFH
• RN License in the state where they are working or covered by compact
• ACM CCM or ANCC Nursing Case Management board certification preferred

Responsibilities

• Identifies and prioritizes patients in need of care management services using a holistic biopsychosocial approach.
• Plans with patients, caregivers and healthcare team to maximize outcomes and cost-effective care.
• Completes documentation reflecting plan of care and handovers at transitions.
• Supports advance care planning, length of stay management and readmission prevention.
• Promotes proactive care and removes barriers to timely testing and treatment.
