CVS Health is seeking a compassionate Registered Nurse Case Manager to support Northern Virginia. This field-based role blends in-person visits with telephonic outreach to deliver coordinated, patient-centered care.

Schedule: Monday-Friday, 8:00 AM - 5:00 PM EST. As a Nurse Case Manager, you will coordinate care, remove barriers, and support better outcomes across providers and services, while reviewing claims history and ensuring regulatory compliance.

Qualifications

Active, unrestricted RN license in Virginia.
Residence in Northern Virginia.
Willing to travel up to 75% with mileage reimbursement.
3+ years of clinical nursing experience.
Experience in case management is a plus.

Responsibilities

Conduct comprehensive assessments (telephonic and in-person) of members' medical, behavioral, and social needs.
Develop and implement personalized care plans to promote short- and long-term wellness.
Coordinate care across providers, programs, and services for a seamless member experience.
Use clinical expertise and data insights to identify risks and gaps in care.
Support members with complex conditions through holistic, whole-person care.
Facilitate referrals to clinical and community resources.
Participate in case conferences with multidisciplinary teams to optimize care strategies.
Review claims history and benefit eligibility to inform care planning.
Engage members to encourage active participation in their care.
