Heart to Heart Hospice is seeking an experienced RN Case Manager to perform initial and ongoing assessments, evaluate patient needs, and implement the individualized plan of care in collaboration with the care team.

You will coordinate with physicians, manage on-call coverage, ensure timely documentation, and promote compliant, compassionate care for patients at end of life in the community.

Formación

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License and reliable transportation with auto liability insurance.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsabilidades

• Perform initial and ongoing comprehensive assessments of patient needs across physical, psychosocial, environmental aspects and ADLs.
• Implement the individualized plan of care and revise the plan as necessary.
• Collaborate with patient, caregiver, physicians, and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning with emphasis on prioritizing needs.
• Provide on-call responsibilities and on-call service to patients and families as assigned.
• Submit all documentation timely and ensure compliance with laws and policies.
• Perform other duties as required and ensure regulatory compliance.
