Heart to Heart Hospice seeks an RN Case Manager to lead comprehensive patient assessments and develop individualized care plans in a hospice setting. You'll collaborate with families, physicians, and the care team to coordinate services across disciplines.

The role requires active RN licensure in Florida, a valid driver's license, and at least two years of RN experience in healthcare or clinical settings. This on-site position offers full-time benefits and on-call responsibilities as needed.

Formación

• Graduate of an accredited nursing program (Diploma/Associate/Baccalaureate).
• Current State RN license.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsabilidades

• Conduct initial and ongoing comprehensive assessments of patients' needs relating to terminal illness.
• Implement and update the individualized plan of care.
• Coordinate care among patient, caregivers, physicians, and other healthcare professionals.
• Attend IDG meetings and participate in care planning and prioritization.
• Provide on-call responsibilities and support to families as assigned.
• Ensure timely documentation and compliance with laws and policies.
• Perform other duties as required.
