Qualifications

• Two (2) years RN work experience required.
• ACMA Case Manager certification preferred.

Responsibilities

• Assess, plan, implement, coordinate, monitor and evaluate care options.
• Manage utilization and metrics to improve patient outcomes.
• Develop patient-centered care plans with a multidisciplinary team.
• Monitor laboratory results and diagnostic data to adjust treatment.
• Maintain documentation for all case management activities.
• Apply benefits and utilization management as a resource for patients.
