Qualifications

Responsibilities

• Review past history
• Conduct case screening within 24 hours of admission
• Interview the patient and significant others to identify health and psychosocial needs
• Review current status and treatment plan
• Identify barriers to wellness
• Determine patient resources
• Determine availability of financial means/insurance benefits
• Evaluate post-acute care environment for accessibility and adaptive needs
• Assess age-specific and special needs
• Assess teaching needs of patient and family based on goals
• Develop individualized care management plan with patient and family input
• Screen options to meet patient needs
• Identify problems and appropriate solutions
• Identify targeted length of stay based on morbidities
