Job summary

• Must have a nursing degree and be eligible for RN licensure in South Carolina.
• Minimum two years of full-time direct care experience and/or hospital-based case management experience.
• Proficiency with MS Word, Excel, PowerPoint and Access; strong planning and organizational skills.

Responsibilities

• Reviews patient admissions within one working day to determine appropriateness of hospitalization using intensity of service and severity of illness criteria.
• Conducts case screening within 24 hours of admission.
• Interviews patient and family to identify health and psychosocial needs; reviews treatment plan and resources.
• Determines availability of financial means/insurance benefits; evaluates post-acute care environment for accessibility.
• Develops an individual care management plan with patient and family input.
• Screens options to meet patient needs and identifies services and funding options.
