Integritus Healthcare is seeking an experienced RN Case Manager to oversee home health services in the Berkshires. You will perform Oasis assessments, develop plans of care, and coordinate with primary physicians to ensure safe, effective home-based care.

Requirements include an active Massachusetts RN license, CPR certification, and a valid driver’s license; a bachelor’s degree is preferred. The role emphasizes compassionate care, timely documentation, and teamwork with families.

Qualifications

• 1–2 years of healthcare experience (home health preferred).
• RN licensure in Massachusetts is required.
• CPR certification is required.
• Associate degree in nursing required; bachelor's degree preferred.

Responsibilities

• Assess patient/family learning needs and adjust information delivery.
• Develop individualized care plans with patient and family.
• Coordinate with healthcare providers and community resources for care and discharge.
• Follow physician orders per patient care policies.
• Reconcile medications with patient and physician.
• Manage assigned caseload within the care team model.
• Create daily work plans based on patient priorities.
• Triage visits, messages, and calls by priority and urgency.
• Demonstrate strong assessment and care planning skills.
• Supervise Home Health Aides per state/federal requirements.
• Maintain knowledge of pharmacology and medication reconciliation.
• Communicate with all disciplines in the home care setting regularly.
