Job summary

Mt. Graham Regional Medical Center is seeking a dedicated Case Manager to coordinate patient care across the continuum under the supervision of the RN Manager Case Management.

The role focuses on planning, coordinating, and monitoring care for the assigned caseload while ensuring efficient use of resources and adherence to established standards of care. The Case Manager will document variations in patient needs and ensure timely achievement of outcomes within an appropriate length of stay.

Qualifications

• Must be a graduate of an approved school of nursing.
• Proficiency in English with excellent written and verbal communication.
• Experience in RN acute care preferred.

Responsibilities

• Coordinate daily planning, coordination, and monitoring of patient care across the continuum.
• Develop care on assigned caseload and ensure effective use of resources.
