Qualifications

• Bachelor's Degree in Nursing; Masters preferred.
• 3-5 years RN experience in acute care; homecare/long-term care or utilization review preferred.
• Current NYS RN license.
• CCM certification preferred; re-credentialing of CCM status expected.
• PRI Certification to be obtained within 1 year.

Responsibilities

• Review new admissions to identify patients needing UR, discharge planning, and/or case management to optimize outcomes and reimbursement.
• Perform continued stay reviews using standardized criteria to justify inpatient stay.
• Collaborate with physicians and clinicians to expedite diagnostic testing, treatment and consultations.
• Document all clinical reviews in Case Management Documentation system.
• Discharge planning: assess, coordinate, and monitor the process; obtain post-discharge authorizations; assess PRI for residential facility placement.
• Collaborate in appeals with interdisciplinary team and managed care companies.
• Develop and maintain effective relationships with interdisciplinary team and managed care organizations.
• Participate in nursing orientation and interface with other Medical Center departments.
